Media Consent and Milton Youth Soccer

Milton Youth Soccer (MYS) encourages parents/guardians, families, coaches, and players to make an informed decision as to whether you will allow MYS to use visual images, and/or video recordings, and/or audio recordings and/or written materials that reveal your identity. You do not have to agree to sign this form in order to participate in recreational or travel soccer with Milton Youth Soccer.

By signing our Media Consent Form, you may authorize Milton Youth Soccer to reveal your identity through the use of photographs, video recordings, audio recordings, or written statements.

MYS may use the items above for the following:

• Telling and teaching people about Milton Youth Soccer in materials that are produced by Milton Youth Soccer.

• Public presentations (such as teaching a class) that may occur within Milton Youth Soccer or other locations, such as a professional conference, tournament, soccer jubilee, or town meeting.

Milton Youth Soccer uses many types of publications, its website and its social media channels (such as Facebook and Twitter) for telling and teaching people about the organization.

Please understand news organizations might use the photographs, recordings, or written material in stories that appear locally or nationally. These materials could be used on radio and television programs, internet websites, social media sites, or in newspapers and magazines. Milton Youth Soccer has no control over news stories produced by outside organizations.

If you are having my picture taken, being recorded, or being interviewed, you have the right to stop things at any time.

You do NOT have to sign our Media Consent Form to participate in youth soccer programs.
